All of the different categories (except the inquiries not initiated by you), are reported to the bank or financial institution when they request a ChexSystem report.

In addition, it is possible to add a short statement to your ChexSystem file, much like you can add to the credit reports.

 

If you feel that the circumstances regarding your information on the ChexSystem report are not fully captured by the information reported, then you should add a statement explaining what actually happened. Here is a breakdown of each of the different sections that will provide you with what is in that section.

Name and Address: This is the name and address that the ChexSystem has on file for you. When you order your report, check this carefully to ensure that they have the correct information in their system. For example, if you recently changed your name or moved to a new address, they may not have the correct information. Note: You can obtain a free copy of your ChexSystem report every year – the next section will show you how to do this.

Reported Information: This section provides the detailed information of all accounts that have been either mishandled, reported, or have outstanding debts. For each financial institution that reports you, they will provide the following information:

• Source of Information (this is your bank)
• Reported Name (this is your name)
• Reported Address (this is your address)
• Reported SSN/ID (this is your social security number)
• Drivers License (this is your driver’s license number)
• Joint Name (this is the name of anyone who is a joint account holder)
• RTN and Account# (this is the bank’s routing number and your
account number)
• Date Reported (obviously, this is the date the information was
reported to the ChexSystem. It will remain on your report for five
years from this date.)
• Reported For (this is the reason you were reported)
• Debt Reported (this is the amount that is owed to the financial
institution. If you have paid the debt, there will be another line that
states the date that the debt was paid in full.)
